-Training
-Evaluation
-Identifying milestones
-effective communication
Through identifying these factors, both companies and outsourcing vendors would have an idea on what area to work on as well as the different strategies to make it more effective.

I could not agree more with communication being the key. And of your list, I find 'expectations; to be the main factor. PRIOR TO any sourcing relationship, having and communicating expectations in every area of the process is paramount - and lack of, the largest contributor to failure.
